Community Palliative Care
Supporting people living at home with life limiting illnesses
Referral FormThe Community Palliative Care service supports people who are living with a life limiting illness as well as their families. Our team visits people at home, in nursing homes, or wherever they live. Our aim is to help patients to be as well as possible at home, for as long as possible.
Most of your visits will be from one of our nurses, known as Clinical Nurse Specialists (CNS) in palliative care. They work closely with your GP and Public Health Nurse to advise and support you and your family at home. Your GP is still responsible for your medical care when you are at home, and any medical decisions by our team will be made along with your GP.
The CNS can provide information, advice and support to patients, families and health care workers in the community. The role of the CNS is especially important when it comes to helping with complex symptoms you may have. They have access to specialist medical advice from our hospice doctors.
Other services of St Francis Hospice, such as our Outpatient and Day Service and In-Patient care, can be accessed through the Community Palliative Care Team.

Our team is made up of doctors, nurses, physiotherapists, chaplains, social workers, volunteers and administration staff. We use our specialist knowledge and experience to work along with the care of your General Practitioner (GP) and Public Health Nurse (PHN).
The service covers all of Dublin city and county, north of the river Liffey, and parts of the surrounding counties.

- Visiting service is available from 8:00 am – 5:00 pm; 7 days a week
- Telephone advice is available from 9:00 pm – 8:00 am from the in-patient unit staff at St Francis Hospice Dublin
